LPF Exam Qualification Form

IF you didn't pay for your exam in advance, you will not have a A PRE-PAID EXAM TAG. To enable us to better serve you, we strongly recommend that you "SIGN IN", first. Then return to this form and fill it out. All credential holders must have a profile. If you don't, you will need to register, first, before filling out this form.

This form is required to successfully qualify for all exams.
After completing this form, you will be shown a form submission confirmation. If you don't have a pre-paid exam, follow the instructions to take the next step. If you have a pre-paid exam, then print out your submission confirmation and wait for the LPF to contact you with your next step. Submitting this form more than once, will only serve to delay the process.

Choose which exam you wish to qualify for:


IF your exam was paid for in advance of you filling out this form (For example, if you purchased the exam from the LPF store; if your company purchased your exam for you. Or if someone awarded you an exam scholarship.) In other words, your exam is "Pre-paid", then pick "Pre-paid LPQ or Pre-paid LPC". 

BE ADVISED: IF your exam is paid for in advance and you mistakenly enter LPQ-########## or LPC-########### (these are pre-paid course enrollment keys and not pre-paid exam tags), instead of LPQEX-987654321 or LPCEX-987654321 (pre-paid exam qualification tag examples) your voucher will NOT be issued. The LPF will NOT notify you that you entered in the wrong pre-paid tag.

Pre-paid exams are exams that have been paid, in advance, by your company, a scholarship, or covered by the LPF. The pre-paid exam tag for the LPQ Exam or LPC Exam would have been issued to you by the LPF prior to you filling out this form.  They look like "LPQEX-999999999" or "LPCEX-999999999". If your exam is pre-paid and you do not have a tag like the example, you should! Please contact us for further action.
Qualification Question No. 1: Have you ever been convicted of a felony, or if in military service convicted by a general court martial?
Qualification Question No. 2: Have you ever been convicted of a misdemeanor involving moral turpitude ("moral turpitude" means an offense that calls into question the integrity or judgment of the offender, such as fraud, bribery, corruption, theft, embezzlement, solicitation, etc.)?
Qualification Question No. 3: Are there any criminal charges (felony or misdemeanor involving moral turpitude) now pending against you?
Statement 1 (S1) I certify, to the best of my knowledge that the answers to the questions above are true. I realize that falsification of information on this application will result in the forfeiture of the registration fee I have paid.
Statement 2 (S2): I certify that I am a High School Graduate or have been issued a GED diploma.
Statement 3 (S3): I understand as LPQ I do not recertify, but as an LPC, I know I must re-certify every 3 years after passing my exam, to maintain my credential. I have read, carefully, the information on recertification, and I have all the information I need to maintain my LPC, should I choose to do so.
Statement 4 (S4): I certify that I have not taken this exam previously, for at least 30 days. AND I am either a U.S. Citizen OR I have purchased and taken the FULL LPQ or LPC course (depending on which exam you are qualifying for).
Statement 5 (S5): I certify that I agree to the terms and condiitions of holding the professional designation of LPQ/LPC (whichever applies) and will abide by those terms and conditions during the course of my use of the LPQ/LPC (whichever applies) credential.
Statement 6 (S6): I am now aware that after passing the exam, the LPQ designation will not expire while the LPC designation will expire in 3 years. In order to keep a LPC credential beyond three years, I need to participate in the LPF recertification program.  I will either earn and sends 20 Continuing Education Units to the LPF each year or sit for the exam again prior to its expiration date.

Eligibility Requirements for the LPQ and the LPC:

  • I certify that I will abide by the LPF's code of ethics.
  • I certify that I meet the minimum requirement for the LPQ.
  • I certify that I will not share any of the exam questions with others, after I sit for the exam.
  • I certify that I will use the LPQualified(LPQ) or the LPCertified (LPC) designation appropriately, such as using it only after successfully passing the exam. For LPC, while my credential is active.

*I certify that if I am not a U.S. Citizen, I have taken the LPQ exam prep-course (for LPQ exam) or the LPC exam prep-course (for LPC exam) before qualifying for the exam I am now submitting this form for.

General Eligibility:

  • I certify that I have a high school diploma or a GED equivalent.
  • I certify that I have three years of retail loss prevention and or asset protection experience.


  • I certify that I passed the LPQ exam AND have 1 of the 3 below:
    • 3 years of business experience
    • Bachelor Degree
    • Master's Degree
